What Is the Forefoot?
Anatomically, the human foot is divided into three main sections: the hindfoot (your heel and ankle), the midfoot (your arch), and the forefoot (the front part of your foot).
Despite taking up a small amount of space, the forefoot is a complex system packed with structural elements:
- 19 Bones: This includes the 5 long bones running through the middle of your foot (metatarsals) and the 14 smaller toe bones (phalanges).
- Soft Tissues, Joints, and Tendons: An intricate web of ligaments, tendons, fat pads, and joint capsules holding everything together.
Why Is the Forefoot So Important?
Your forefoot is essentially the springboard for your entire body. It carries roughly half of your body weight when you stand still. More importantly, during the propulsion phase of walking or running the final 30% of your stride where you push off the ground the forefoot is the only part of your foot touching the ground.
Because it acts as a lever to drive you forward, any issue in your forefoot can ripple upward, affecting your ankles, knees, hips, and lower back. Likewise, poor alignment further up your leg can throw extra stress onto your forefoot, leading to pain and structural changes over time.
6 Common Forefoot Conditions Seen in Podiatry

1. Metatarsalgia (Ball-of-Foot Pain)
Metatarsalgia is a general term for pain and inflammation in the ball of the foot (under the heads of your metatarsal bones). It often feels like walking on a sharp pebble or a folded over sock.
- Why it happens: It is typically caused by repetitive stress or overloading on the front of the foot.
- Common triggers: High-impact sports, wearing high heels or unsupportive shoes, thinning of the foot’s protective fat pad, or structural issues like bunions and hammer toes.
2. Bunions (Hallux Valgus)
A bunion is a visible, bony bump that forms on the joint at the base or side of your big toe. (A smaller bump that forms on the joint of the pinky toe is called a Tailor’s bunion or bunionette).
- Why it happens: Bunions develop slowly over time. Continuous pressure forces the big toe to angle inward toward the second toe. This pulls the joint out of alignment, stretching ligaments and tendons and causing the bone to protrude outward.
3. Hammer Toes
Hammer toe is a common deformity where one or more of your smaller toes bend abnormally at the middle joints, pointing downward like a claw or hammer.
- Why it happens: It stems from a muscle imbalance in the foot where stronger muscles overpower weaker ones, pulling the toe joint out of position.
- The impact: Because toes play a major role in balance, hammer toes alter how you walk. They often rub against shoes, causing painful calluses, corns, and cosmetic changes. (Depending on which joint bends, they fall into three types: classic hammertoe, mallet toe, or claw toe).
4. Morton’s Neuroma (Interdigital Neuroma)
If you feel a burning, tingling, or stabbing pain between your toes or feel like you’re standing on a pebble you might be dealing with a neuroma.
- Why it happens: A nerve in the forefoot becomes compressed or irritated by surrounding bones and ligaments. Over time, the nerve tissue thickens and swells. This most commonly occurs between the third and fourth toes (accounting for 64% to 91% of cases).
5. Sesamoiditis (Hallux Sesamoid Syndrome)
Underneath your big toe joint sit two tiny, pea sized bones called sesamoid bones. Embedded inside tendons, they act like pulleys to smooth out toe movement and absorb weight.
- Why it happens: High-impact activities, frequent jumping, or repetitive pressure can inflame the sesamoid bones and surrounding tendons, leading to severe pain right under the big toe joint when you push off.
6. Turf Toe (Plantar Plate Injury)
“Turf toe” is a painful sprain to the main joint of the big toe, specifically injuring the plantar plate a thick, supportive ligament pad underneath the joint.
- Why it happens: It occurs when the big toe is forcefully bent upwards beyond its normal range of motion (hyperdorsiflexion), such as pushing off aggressively in sports like cricket, football, soccer or running.
